Your First Visit

We would like to thank you for choosing Associates in Neurology and will strive to make your appointments here as seamless and organized as possible. We realize your time is valuable and will do our best to be on time for your appointments. In order to help us, please arrive to your visit at least 15 minutes prior to your appointment to complete registration. We ask that you bring a photo ID, your insurance card(s), an updated list of medications you are taking and any previous medical records that you can provide. Please click below if you would like to download our New Patient Registration Form and Financial Policy. We ask that you complete the registration form (plus any additional forms that are appropriate) and bring with you to your appointment.


We are participating providers for many different insurance plans including Medicare, Anthem and United Healthcare. Please contact our office for a complete list or you may contact your insurance company to verify participation.

Patients are required to know the benefits and eligibility requirements of their individual plans. We are happy to assist with verification but request that any specific questions regarding insurance be directed to our billing department prior to your appointment so that we can assist you. We ask that co-payments be paid at time of service. Please contact our billing department with any questions you may have at 219-462-0407.

Appointment Cancellations

While we realize that unforeseen events may cause an appointment to need to be rescheduled, we do ask that notification is given. We require a 24 hour cancellation notice for any appointments with a physician or for testing scheduled in any of our offices. A fee may be charged if proper notification is not received.


When calling our office to renew prescriptions, please call at least 3 – 4 days before you are out of your medication. Please be ready to provide the correct name and dosage of the medication as well as pharmacy name and phone number.
